Home Health Case Manager, RN

ImpactBio has partnered with a Philadelphia based Home Health Care Agency to support them with their staffing needs. This is a Full-Time Direct placement position. The Home Health Case Manager, RN will treat patients in Philadelphia and the surrounding area. This is an incredible opportunity for an experienced individual to have the ability to transfer into the role of Assistant Nursing Director once you’ve proven yourself.

Duties and Responsibilities:

• Maintain proper OASIS documentation
• Ensure all cases are documented correctly using paper based system
• Maintain a professional appearance and demeanor at all times
• Interact with all clients in a respectful way at all times

Qualifications / Requirements:

• Registered Nurse, RN with active license in good standing in the state of Pennsylvania
• Minimum of 2-5 years of prior home health experience with a Medicare certified agency
• Prior OASIS documentation and Medicare certification experience
• Ability to coordinate, implement, and assist in Medicare certification
• Excellent written and verbal communications skills
• Empathy and excellent bedside manner
• Reliable transportation
• Active driver’s license in good standing
